💡 Why EXCELLENCE
how we redefined FMEA management
When we started developing the FMEA EXCELLENCE platform, we asked ourselves a simple but essential question:
What does excellence in FMEA really mean?
To find the answer, we didn’t start with technology — we started with the people who use it.
🔍 Market research – where FMEA really stands today
Between 2022 and 2024/25, we conducted research among 80+ companies — Tier 1, 2, and 3 automotive suppliers.
The results were clear, yet concerning:
Only 10% of companies use specialized FMEA software.
As many as 90% still perform FMEA analyses in Excel spreadsheets.
None of the surveyed companies have experience with AI-integrated solutions.
The majority do not include the costs of failures, prevention, or detection in their analyses.
These findings confirmed what we had seen for years in practice — that FMEA is still viewed primarily as a document, not as a decision-making tool.
⚙️ Main challenges of the FMEA process in industry
Based on our research and discussions with quality engineers, managers, and auditors, we identified the most common pain points in how FMEA is conducted today:
1️Too much manual work and dependence on Excel
– multiple versions, formula errors, duplicated data, poor reuse of past experience.
2️Lack of real-time updates
– changes are not instantly visible, and teams often work on outdated data.
3️Disconnection between DFMEA, PFMEA, and Control Plan
– difficulties in maintaining consistency throughout the AIAG & VDA methodology.
4️Frequent non-compliance with IATF 16949 and AIAG & VDA guidelines
– missing 7-step structure, undefined priorities, and unclear responsibilities.
5️A lengthy and often inaccurate process of defining S, O, and D ratings
– teams assign scores “by feeling,” based on static tables, without real data context — resulting in subjective and inconsistent risk assessment.
6️Lack of a living knowledge base and unused Lessons Learned
– insights from previous projects are not systematically captured or reused.
7️Limited awareness of risk-related costs
– costs of failure, prevention, and detection are rarely included in decision-making.
8️No connection between FMEA and actual process performance
– FMEA often remains theoretical, with no feedback from real production data.
9️No support for higher-level cause-and-effect analysis
– focus remains on tabular input, not on strategic insight.
🧠 Our approach to building the solution
From the very beginning, we knew that excellence cannot be achieved by automating existing problems.
That’s why we decided to change the very philosophy of the FMEA process — transforming it from a passive document into an intelligent decision-making system.
Our team includes:
🎓 PhD-level FMEA experts from the automotive industry,
📜 AIAG-certified specialists,
🏭 engineers with years of hands-on experience in operational FMEA roles,
📚 consultants and trainers who have led numerous workshops on FMEA and risk management,
📊 and authors of more than 20 scientific papers on risk, quality, and industrial AI applications.
We have been active in this field since 2014, when we began conceptually developing the idea of digitizing and improving the practical implementation of FMEA in the industry.
Over the years — through operational work and consulting projects — we have closely followed the evolution of methodologies, standards, and industrial needs, gradually shaping our vision.
In recent years, we turned that vision into reality: we developed the application, tested it with industrial partners, and in February 2025 launched FMEA EXCELLENCE — a platform that finally connects people, data, and decisions into one digital ecosystem for risk management.
🚀 Our solutions that deliver excellence
✅ AI analytics and FMEA-BOT – suggests causes, effects, controls, and risk priorities using LLM and RAG technologies.
✅ Integrated time and financial factors – evaluates the impact of risks on cost, resources, and deadlines, giving management a clear view of real business consequences.
✅ Real-time collaboration – all participants see updates simultaneously; no more “v2_final_final.xlsx” files.
✅ Compliance with AIAG & VDA methodology – guides users step-by-step through all seven FMEA phases, ensuring full compliance with IATF 16949 requirements.
✅ Knowledge base and Lessons Learned – the system remembers successful solutions and automatically suggests them when similar issues arise in new projects.
✅ Automatic action tracking – transparent monitoring of preventive and detection activities, with notifications and progress tracking.
✅ Advanced reporting and KPI dashboards – real-time insight into project risks, status, and results, with instant generation of analytical and financial reports.
✅ Expert system for automatic risk definition –
FMEA EXCELLENCE features a built-in expert logic engine that, based on input data about functions, potential failures, and effects, automatically recognizes patterns, asks follow-up questions to the team, and accurately defines key risk parameters:
• S (Severity) – the seriousness of the effect,
• O (Occurrence) – the likelihood of the failure occurring,
• D (Detection) – the ability to detect the issue before it reaches the customer,
• and AP (Action Priority) – the priority of actions based on combined risk and real impact on cost and timing.
This expert system delivers more precise and consistent risk evaluations than traditional team-based scoring, eliminating subjectivity and accelerating the process severalfold.
📈 Results we achieve
Our goal was never just to digitalize FMEA — it was to deliver measurable, sustainable improvements that bring real value to teams and organizations.
Today, companies using FMEA EXCELLENCE experience tangible benefits in their daily operations:
⏱️ Up to 60% faster creation and maintenance of FMEA analyses
– enabled by automation and the expert system that eliminates repetition and manual effort.
💰 10–40% reduction in hidden costs during development and production
– through better cause identification, more accurate prioritization, and fewer repeated issues.
🎯 More accurate priorities and more reliable decisions
– powered by AI analytics that, beyond Action Priority (AP), incorporates financial, time, and strategic dimensions of every risk — giving management a complete view of project and business impact.
🤝 Improved collaboration between engineering, quality, and management teams
– all stakeholders work in real time, sharing a unified view of risks and clearly defined responsibilities.
📏 Full compliance with customer requirements and relevant standards
– the system fully adheres to AIAG & VDA FMEA methodology and supports IATF 16949 requirements, enabling companies to meet OEM expectations and audit criteria without compromising efficiency.
🔒 Reduced likelihood of customer complaints and product recalls
– through early detection of systemic issues and a proactive focus on prevention before they reach the end user.
🛡️ Reduced safety and compliance risks
– through systematic monitoring of causes and effects that could compromise product safety or regulatory and customer compliance.
